We will need the following information in order to set up your business credit account.
Please provide a copy of the following:

• Retail Business License
• State Tax License
• Federal Tax ID Number
• Store Name and Telephone Number
• Download Forms -> Completed Terms, Credit Application, Trade Reference
• Email completed form to customercare@palmsfashioninc.com
